- In all divorce cases, you are required to complete the following Insurance Notification form to notify your spouse of certain rights regarding health insurance. Note that you must complete the form even if you do not carry health insurance or if each spouse carries his or her own insurance.

- If your case involves the parenting and support of children under the age of 18, we may need to submit a Parenting Plan. Click on the following link, then fill in the form with your desires for the parenting and support of your child/children during the pendency of the divorce case. We will use your form as a basis for discussion before finalizing it for the Court.
